Paycheck
To enroll in direct deposit for your paycheck, you can either contact your employer or download and return the direct deposit authorization form below.
Private Annuity
If you’d like to deposit a regular payment from a private annuity or fund, please complete the direct deposit authorization enrollment form below.
Social Security or VA
If you are still receiving your Social Security, VA or other federal benefit funds as paper checks, you are now required by the U.S. Treasury Department to switch to electronic payments.
